WHAT TO EXPECT DURING the initial VISIT

Imagine a medical appointment in an inviting setting where you can tell your story and have a doctor synthesize an assessment and plan that integrates relevant medical information such as:

  • Family History
  • Social History
  • Mental Health
  • Movement and exercise
  • Nutrition
  • Supplements
  • Sleep
  • Sexual Health
  • Preventive Health

We will have time to answer your questions, address your concerns, engage in shared decision-making and co-develop a plan that works for you.

The plan may include:

  • Blood work
  • Imaging
  • Referrals to other specialists
  • Hormonal and Non-hormonal prescriptions
  • Goals for movement, nutrition, sleep, and/or intimacy

The goal is for you to feel more informed and empowered to achieve your wellness goals by the end of your visit.

WHAT TO EXPECT for ongoing care

At the end of your first visit, we will schedule a brief phone check-in for 6-10 weeks after starting any new medications.

Your treatment plan will depend on how you are feeling and your health goals.   On average, hormone balance is achieved in 2-4 hours of face-to-face time over the course of the first year.

We will schedule your follow-up visits for the right amount of time to meet your needs, whether that be a phone, virtual, or in-person visit.

You will have access to self-schedule or message us through the Patient Portal (ChARM PHR) throughout the year.

Every 12-months, we will meet for an Hormone Strategy Review ($300), at which time Dr. Duncan will coordinate the appropriate number of refills for any prescriptions she provides. You will be billed $30 monthly for 10 months which covers an additional 45 minutes of synchronous care (phone, video, in-person) and unlimited asynchronous messaging.

All new patients start with the same three-month onboarding process:

60-minute face-to-face visit to review your history, symptoms, and goals. Includes chart review and individualized plan development.

    Asynchronous care  (First 3 months)

    Care Tailored to Your Needs

After your first follow-up, you and Dr. Duncan will decide together whether ongoing support through membership care or occasional pay-per-visit care is the best fit for you.

  • Membership Care (Months 4-11)

Why Membership Works for Most Patients

    • Predictable costs — one flat fee covers most ongoing care
    • Proactive support — The right care at the right time, so you and Dr. Duncan can focus on your health
    • Priority access — members have direct access to Dr. Duncan through messaging.

Your investment:  $700 total (billed as $87.50/month for 8 months)

    Includes:

    • 2 hours of additional visit time (in-person or telehealth, used as needed)
    • Ongoing portal messaging for brief updates and questions.  If warranted, clinical care decisions without a "formal" visit, as appropriate.
    • Lab review and prescription coordination

Additional time beyond 2 hours, if needed, is billed at $125/visit**

    (Non-Member rate $250/visit)

This model offers predictable costs, continuity, and flexibility — you can reach out when you need support without worrying about per-visit charges.

**A standard follow-up visit is scheduled as 30 minutes, but may be more or less  time depending on need.

  • Non-Member (Pay-Per-Visit) Care

At your first follow-up appointment, you and Dr. Duncan will discuss if membership or pay-per-visit is right for your needs.

Pay-per-visit may be right for you, if

    • You are young and healthy
    • You find a regimen early on that achieves your goal(s) of feeling your best self.
    • You have reliable access to primary care to provide additional support, such as ordering sex-specific, age-appropriate studies based on the most up-do-date menopause-informed recommendations.

If you decide to schedule visits as needed, you will be billed $250/visit**

This option works best for one-time or occasional consultations

    •  If you anticipate < 3 visits/year  ($750 total cost)
    • Limited portal messaging for non-clinical issues (ie - prescription refills without adjustments)
    • All clinical decisions will require a visit.  Members have the benefit of asynchronous care through portal messages, as appropriate.

**A standard follow-up visit is scheduled as 30 minutes, but may be more or less depending on need.
